What this page collects
When you submit the callback form on get.chiroportllc.com, Chiroport LLC ("we") receives the information you choose to enter: your name, phone number, email address (optional), how you'd prefer to be contacted, when to reach you, and your preferred language.
We deliberately do not ask for clinical or medical details on this form. Any such information is collected during your appointment or callback through our regular intake process, where it receives the protections required by HIPAA.
What we do with it
- Reach out to you about your inquiry — by call, text, or email, based on your preference.
- Keep a record of your inquiry inside our clinic systems.
- We do not sell, rent, or trade your information.
- We do not share your information with third parties for advertising.
- We will not share, sell, rent, or otherwise disclose your mobile phone number, SMS opt-in data, or any consent records to any third party or affiliate for marketing, advertising, or promotional purposes. This restriction applies even if you opt out of SMS messages later.
Cookies and tracking
This page does not set advertising or analytics cookies on your browser. We do not run a Meta Pixel or other client-side ad tracker on this page. If we measure ad performance in the future, we do so with server-side, privacy-preserving methods that do not transmit identifying health information to ad platforms.
SMS / text messaging
The SMS program is optional. On our callback form you must affirmatively check the SMS opt-in box and provide your mobile number to consent to receive SMS messages from Chiroport. Submitting the callback form alone does not enroll you in SMS — you can request a callback without opting in to texts, and we will only call you. If you check the SMS opt-in box, you consent to receive SMS messages and calls from Chiroport regarding your inquiry, appointment, and care.
Message frequency varies based on your interaction with us — typical recipients receive between 1 and 10 messages per month (for example: callback confirmation, appointment confirmation, 24-hour appointment reminder, missed-call recovery, and post-visit follow-up). Message and data rates may apply from your wireless carrier. Chiroport does not charge for SMS messages.
You can stop messages at any time by replying STOP to any message. Reply HELP for help, or call (651) 202-3178. Replying STOP will unsubscribe you from further SMS messages; you can reply START to opt back in.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors that support Chiroport's services (for example, our SMS delivery provider) is permitted as needed to deliver the messages you have requested. All other use categories — including lead generation, advertising affiliates, marketing partners, and data brokers — are excluded from sharing.
Your choices
You can ask us to delete your inquiry record at any time by emailing chiroportllc@gmail.com or calling (651) 202-3178.
Health-information privacy (HIPAA)
Once you become a patient — or share clinical information with us in any form — that information is protected health information ("PHI") and is governed by our Notice of Privacy Practices, in addition to this notice.
